HOUSING OPPORTUNITIES FOR PERSONS WITH AIDS (HOPWA)
CASE MANAGER

POSITION SUMMARY: The HOPWA Case Manager provides social work and client advocacy services to clients of Big Bend Cares. This position is directly responsible to the Director of Client Services.

QUALIFICATIONS: Bachelor’s degree in human services and two years experience or a Masters degree in human services. Knowledge of HIV disease, government programs, and medical social work procedures is preferred.

RESPONSIBILITIES:

§ Ensures completion of the required application procedures.
§ Ensures the linkage of clients with needed programs.
§ Monitors follow through on referrals and appointments.
§ Has thorough knowledge and understanding of the Client Rights and Responsibilities and all client procedures.
§ Advocates on client’s behalf in accessing needed services.
§ Ensures clients’ Statement of Need and HOPWA Worksheet forms are updated upon each request for housing assistance.
§ Ensures that all clients’ HOPWA update forms are completed and given to the client’s primary case manager at least every 3 months unless otherwise stated.
§ Recommends changes in case management policies and procedures.
§ Completes pre-intakes as required.
§ Maintains and reports caseload data as requested.
§ Maintains assigned client files.
§ Completes billing documentation.
§ Acts as the agency liaison for housing related issues.
§ Ensure clients complete Client Satisfaction Surveys specific to housing.
§ Ensure 90 day surveys are submitted to clients as required by HOPWA guidelines.
§ Must adhere to HOPWA guidelines provided by Dept. of Health.
§ Develops housing resources for clients.
§ Researches and locates Low Cost Housing options for clients.
§ Attends Big Bend Homeless Coalition Network meetings.
§ Other duties as assigned.
